Have you tried whether the "load last known workspace state" vs. "load last saved workspace state" (Settings > Interface) makes a difference?

I usually change values in the audio mixer by hovering the mouse over the fader and using [Shift]+[Mousewheel] to in-/decrease the volume in steps of 1 dB. Has the particular file worked with Media Composer before? Did you create it from scratch or was it made by someone else? Have you checked - Mode (8/16/32 bit, RGB/CMYK/Lab etc.) - Additional alpha channels (besides layer transparency) - Embedded color profile
