It seems that one drive failed and it is rebuilding correctly to the hot spare. You can remove the failed drive and replace it with an Avid only branded ISIS drive. Once the rebuild is done (yellow error) and the spare drive has been replaced (red error) then it should clear the status.

If you are still intent on rebuilding the server, then...

NOTICE: THIS WILL DELETE EVERYTHING on the server. NO BACKUP will be created other than what you create yourself. If you need to only rebuild a part of the server, then you need to specify and take a different course of action.

    1. Delete all users and workspaces

    2. Delete the storage group(s)

    3. Unbind the storage engine

    4. Delete the File System

    5. Delete the RAID set

    https://avid.secure.force.com/pkb/articles/en_US/troubleshooting/en385175

    6. Run the Avid ISIS installer

This should initiate a new RAID set rebuild (takes about 25 hours).

AND depending on the version it may require a firmware flash and several reboot of your server.

Once the RAID set is build and the firware has been udpated, you can start a new file system and bind the storage engine.

you need to be able to recover the data drive set metadata, which is stored on the 2nd partition of the boot mirror (the OS is installed on "C:", there is a "Data" partition on "D:")

Every version of ISIS supports every software release AFAIK.

If you are not preserving any workspaces, there is no need to backup the metadata.

As far as I'm aware there was never a change to the dongle or software license for ISIS.

The supported upgrade path is listed in the read me on page 17. If you're running anything earlier to 4.2.2 you will need to step through 4.2.2 prior to installing 4.7.13.

It's a good idea to upgrade the MegaRaid software while you're at it.

Great all upgrade done successfully, but we found that a new hard disk drive failure and now we only have 14 hard disk drives.

Is that possible to create new raid set using only 14 hard disk drives for isis 5000 ? for example create three groups each group have 4 hard disk drives and two hard disk for hot spare ?

The ISIS 5k has 16 drives. 15 are used for three LUNs of RAID 5 - 4+1. The 16th is a global hot spare, you will need 15 drives at least.
